Methods of Payment
We accept cash, personal checks, Visa, MasterCard, Discover, American Express, Debit Cards, Cashiers Checks and Money Orders. Our office charges a $30.00 fee for each returned check.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage. Your insurance policy is a contract between you and your insurance company. It is impossible for us to know every insurance plan and benefits, therefore, it is your responsibility to know your benefits and keep track of your coverage. We require you to notify us of any changes in your insurance at least 48 hours prior to an appointment. Failure to do so will result in out-of-pocket expenses to you. Please be aware that not all insurance companies cover all services.

All estimated co-pays and deductibles are due at the time of service. Any balance not paid by the insurance company within 60 days of the date of service will become the responsibility of the patient. Any patients without insurance must pay in full when services are rendered. We reserve the right to submit any unresolved account balance over 90 days old to a credit reporting company. Unfortunately, we are unable to bill any secondary or supplemental insurance plans.
